As we continue to grow, we are looking to add a qualified, experienced, full time Triage & Care Registered Nurse to our team. In care management you are developing care plans for patients with complex and co-morbid conditions. You will also track hospital admissions and ER visits and provide follow-up in a timely manner. Job duties within the triage role include coordinate patient care, applying nursing assessment skills for telephone triage, portal, and in office care to patients utilizing critical thinking and nursing judgement.
As we continue to grow, we are looking to add a qualified, full time Medical Assistant to our team. Position schedule consists of 4 daytime shifts, with one evening shift and Saturday rotation.
