As an RN performing triage nursing to patients, you will have the opportunity to broaden your nursing skills and experience in a fast paced primary care office setting. This role is an in office position, no remote work. Job duties within the triage role include coordinate patient care, applying nursing assessment skills for telephone triage, portal, and in office care to patients utilizing critical thinking and nursing judgement.
Primary Areas of Responsibilities:
Ensure a high level of patient satisfaction
Maintain ongoing communication, support and education with patients in specific populations
Track hospitalized patients and assure transition of care upon discharge
Assist practitioners in complying the quality of care measures for their patients
Coordinate patient care ensuring that the providers plan of care is carried out within NYS RN scope of practice
Knowledgeable of care management principles and proven proficiency in nursing care management
Administers vaccines in accordance with pediatric and adult guidelines and practitioner orders.
