Wheatfield Location Update
Our lease is expiring at the end of 2024 and after much thought and consideration, Primary Care of WNY has decided to close our Wheatfield office located at 3799 Commerce Court, Suite 100, Wheatfield NY 14120. This decision was not made lightly, and we want to assure you that your health, well-being, and ongoing care are of the utmost importance to us. We value you as a patient and hope you will continue your care at one of our other practice locations.
This change will be completed in two phases.
PHASE 1 – Tuesday appointments at Wheatfield
Effective Tuesday, July 2, 2024, all appointments scheduled for Tuesdays at Wheatfield will be relocated to our Maple office located at 2350 Maple Road, Suite 100, Williamsville, NY 14221. If you have an appointment on any Tuesday beginning July 2nd, we will automatically shift your appointment to our Maple office. If you prefer a different location option or need to adjust that appointment, please send us a portal message or call (716) 839-8000.
PHASE 2 – All remaining appointments at Wheatfield
Friday, November 15, 2024, will officially be our last day of patient visits at the Wheatfield location. If necessary, the timing of closure could change.
If you have an appointment after November 15, 2024, we will contact you soon to shift your appointment to one of our other locations based on the provider-specific changes noted below:

Again, caring for you and your family is important to us, and we are prepared to continue this relationship. However, if you choose to seek care from another practice, you can have your full medical record transitioned to your new provider by completing a “Release of Information” form. To download a copy of this form, click “Release of Information” button in the top left corner.
